This week I've been reflecting on the moment a developer watched an AI agent erase 2.5 years of work in minutes and what it tells every leader about the difference between deploying a tool and deploying something that makes decisions.
The $5 Decision That Wiped 2.5 Years of Work
It started with a reasonable goal and a small cost-cutting instinct. Alexey Grigorev, founder of DataTalks.Club, one of the most respected data engineering communities in the world, and a widely followed voice in AI education, was migrating a small website from GitHub Pages to Amazon Web Services. To avoid the expense of a separate AWS environment, he decided to run the new site on the same infrastructure already hosting his DataTalks.Club course platform. The savings would have been $5-10 dollars per month. The cost of that decision would be 2.5 years of production data and a 24-hour recovery crisis published in a transparent, unflinching post-mortem on March 10, 2026.
Here is what makes this story different from every other AI disaster headline. Before the migration began, the AI agent Grigorev was using Claude Code, an agentic coding assistant that can write code, execute commands, and manage cloud infrastructure on your behalf, explicitly advised him not to combine the two setups. It flagged the risk of running a new migration in the same environment as live production infrastructure. Grigorev acknowledged the warning and told it to proceed anyway. That decision, made in a moment to save a few dollars, became the fault line through which everything else fell through.
The chain of events that followed is a textbook case of how AI agents fail when governance is absent. About 2 million rows of student coursework homework submissions, project records, leaderboard entries across multiple course cohorts were gone in a single command, executed without a confirmation prompt, without a human approval gate, and without a warning that it was about to happen.
Grigorev opened an emergency AWS support ticket before midnight. His standard support plan would not respond fast enough during a production outage, so he upgraded to AWS Business Support on the spot, incurring a permanent 10% increase to his monthly cloud bill. AWS engineers joined within 40 minutes, discovered an internal snapshot that was not visible in the customer console, and escalated the recovery to internal teams. The data came back after roughly 24 hours. It was a close call. For students mid-session in an active course, the loss would have been permanent.
Tools Respond. Agents Decide.
This story is about what happens when the humans in charge of AI do not define the rules before handing over execution authority. And that distinction between a tool and an agent is the most important concept every leader deploying AI right now needs to internalize.
A tool waits for you. You ask it a question, it gives you an answer, and you decide what to do with that answer. A tool cannot act without your explicit next instruction. An AI agent operates on a fundamentally different model. You give it a goal, and it determines the steps to get there, writing code, running commands, modifying infrastructure, executing changes in your live environment — often without pausing for sign-off at each step. That autonomy is the entire value proposition. It is also the entire risk.
Think of it this way. A GPS tool gives you directions and waits for you to drive. A GPS agent books the hotel, reroutes the trip, reschedules the meeting, and charges your card while you are focused on something else. Both have their place. But you would set very different boundaries for each of them before handing over access to your calendar and your finances. The same logic applies to AI agents in your infrastructure.
This is the shift most organizations have not made yet. You are no longer just choosing software. You are choosing a system that can take consequential, irreversible actions in your environment based on its own interpretation of your intent. That changes everything about how you evaluate, deploy, and govern it.
Why Ethics and Security Keep Getting Treated as Version 2 Problems
If you ask most teams when they plan to address AI ethics and security guardrails, the honest answer usually sounds something like this: after we launch, once we see how it performs, when we have more bandwidth, when the product matures. The intention behind that answer is genuine. The logic feels reasonable under deadline pressure. The result is a system running in production with the authority to act on your data and your infrastructure before anyone has formally defined the limits of that authority.
This pattern has a name. It is called move fast and fix it. And it worked reasonably well in the era of software tools, because the failure mode of a bad tool was a bad output you could review and discard.
The deeper problem is cultural. Ethics and security in AI are still widely treated as IT responsibilities or compliance checkboxes, not as leadership decisions with operational stakes. Speed to deployment is framed as a competitive advantage. Safety review slows deployment. So safety review gets compressed, delegated, or deferred to the next sprint. What leaders have not yet priced into that calculation is the full cost of the incident waiting on the other side. Grigorev now pays 10% more for AWS permanently, spent 24 hours in a recovery crisis, and had to write a public post-mortem about the experience. For a solo founder, that is survivable. For an organization handling sensitive health data, financial records, or government systems, that same incident is not likely to look like a cautionary blog post. It looks like a regulatory investigation, a breach notification requirement, and a crisis communications response.
There is something specifically important about the Grigorev incident for anyone leading an AI adoption effort. The AI gave the right advice. It flagged the risk before the migration started. The human overrode it, and there was no organizational or technical governance layer to enforce what the human should have respected on their own. When ethics and security are afterthoughts, you are not just hoping the AI behaves. You are hoping the human deploying it makes every right call in real time, under pressure, late at night, on an unfamiliar machine. That is not a strategy.
The Guardrails Framework: What Should Have Been There First
The good news is that this is a solvable, not a complicated, problem. The framework for deploying AI agents safely is clear and well-established. What it requires is the organizational discipline to treat it as the starting line, not as something to revisit after the first incident forces the conversation.
Scope the access before you scope the task. Before any AI agent is connected to a live environment, answer one question with precision: what is the minimum access this agent needs to accomplish its specific job? Not what is convenient. Not what gives it more capability. The minimum. Grigorev's agent had authority over the entire production infrastructure stack. It should have had access only to the isolated migration environment being built. That single scoping decision, made before the first command ran, would have changed the entire outcome. The principle of least privilege applies to human employees, software systems, and AI agents equally — and without exception.
Put a human back in the loop on irreversible actions. Routine and reversible steps can run autonomously. But any action that cannot be undone, deleting infrastructure, dropping a database, pushing to production, should require explicit human approval before execution. This is an approval gate, and it is the single most effective structural control for agentic AI.
Verify your backups before you need them. Grigorev discovered during the crisis that his automated backup snapshots had been deleted along with the database because they were part of the same infrastructure stack the agent destroyed. He had assumed backups existed. He had not tested the restore path end-to-end. AWS recovered the data from an internal snapshot that was not visible in the customer console, a recovery that was not guaranteed and required an emergency escalation. Your backup strategy is not a backup strategy until you have confirmed it survives the specific failure mode you are most likely to encounter.
Treat the AI's warnings as governance checkpoints. This is the most direct lesson from this incident. The AI flagged the risk before the migration started. Grigorev had no organizational process for treating that flag as a formal review moment, no checklist, no second sign-off, no escalation path. If your team has no defined protocol for responding when an AI agent surfaces a concern, the warning means nothing. Build the process that gives the AI's caution actual weight.
The New Rules of AI Agent Deployment
Scope before you deploy. Define the minimum necessary access and hard-environment boundaries before the agent touches any live system. Access granted is authority given — treat it that way.
Approval gates on irreversible actions. Any action the AI cannot undo requires explicit human sign-off before execution. Every time. No exceptions.
Ethics and security are Day 1 decisions, not Version 2 features. The governance conversation happens before deployment, not after the first incident forces it.
Test the restore path before you trust the backup. An untested backup is an assumption, not a safety net. Confirm your recovery process works in the exact failure scenario you are most likely to face.
When the AI gives a warning, build a process to honor it. Create the organizational checkpoint that gives AI surfaced risk flags actual weight before the human in the moment decides to override them anyway.

The story that matters here is not that an AI agent made a mistake. It is that the human had every signal they needed to prevent it, an explicit AI warning, an incomplete environment, and there was no governance layer strong enough to catch what happened when those signals were overridden. That is a leadership and design problem. And it will keep producing the same outcome until organizations stop treating ethics and security as something they will get to in the next sprint.
AI agents are not tools you point and click. They are decision-making systems you deploy into your environment with a level of authority you define. If you have not defined that authority explicitly with hard technical limits, approval gates on destructive actions, and tested recovery paths, then you have not deployed an AI agent responsibly. You have handed someone a key to your building and assumed they will only go where you would want them to go. Build the guardrails first. Not because you do not trust the technology. Because trust is something you architect, not something you assume.
